In recent years, technological advances have widened the diagnostic and therapeutic possibilities in Differences of Sex Development (DSD). In parallel, societal and ethical values have changed in a way that several aspects of traditional DSD care have been vigorously questioned. In answer to this (r)evolution, new management models, based on transparency, psychosocial support and patient autonomy have emerged, and teams have expanded in order to provide qualitative individualized interdisciplinary care. In spite of initial optimism, DSD teams across the world continue to struggle to understand if and how these models can support a favorable long-term outcome in all children who have a DSD. What works well, and what are the possible pitfalls? What are the new challenges we face, and how can we further improve? While many of these questions currently have no answer, it is clear that we need to intensify collaboration and cross-talk across disciplines more than ever in order to understand the opportunities and limitations of the various approaches and to navigate our patients and families through the many uncertainties they may encounter.
